Help in launching menu with lookup function

Hello All,
Can anyone assist me in launching a menu with a lookup function. I am having problem launching it. Attached is the file

Problem:
I have created lookup file for a householding list and I have household file. In the menu, I want the lookup file to work and confirm entering the listing then procedure to the household file.

Also in the program file, I want it to create single file for individual household that will be interview by using the variables(District, EA, Household, interviewer id)



Would be grateful for assistance.
