Be aware that check boxes only work with "Alpha" values.
The item must be of type "Alpha", and must have a length equal to the number of possible responses. The value set must have that same number of discrete values. Each value corresponds to one of the responses and will be displayed as a separate check box. When the boxes are checked, the corresponding values will be put into the alpha field, from left to right.

You have to:

1) double clic on the Patientpathways.ent file to open it

2) in the form, right clic on the field you want to convert in thext box field (it is set as text box by default) and select Field properties

3) right to "Capture type: Text box" clic on the "Change" button

4) select "check box"

For this to work, you need to have in your value set only one digit or letter for each label. replace all digit by letters (note that it would have been OK with numbers if until 9):

1= a
2= b
3= c
4= d
5= e
6= f
7= g
8= h
9= i
98= j
99= k
