Can This Be Created?

Please I will like to know if the attached image below can be converted into or created in a CAPI application.
There can be many Blocks i.e Block 1, Block 2, Block 3, etc.
I have equally attached a filled example to give a clear picture of what I mean to say. Enumerators will go to the field with blank pages and collect such data. Please will it be possible to design a CAPI appication for this form? If YES, please how do I go by it?

Yes. There are many CSPro examples provided with our installation. Look at the population_record in the household.ent, it's essentially what you want to do here.
