Template of questionnaire

Hi
Can you please recommend a questionnaire template that may help CAPI developers to easily identify item type (Alpa or Numaric), item length, applicable skips, range for answer (value) errors messages, etc etc. Actually i want to give it to my questionnaire developer to adopt and then i can easily identify and develop CAPI application in CSEntry.

Here's an Excel template that I have used for a few questionnaire specification workshops. Each time I tweak it, so I would describe it as a work in progress.

When the participants begin I will have them create a new sheet for each question and paste the template into each. I then have them think about each question and have them fill out the template for each. Feel free to use and modify for your own needs.
