Why are some cases not processed?

As far as I am aware, when you do a breakout process, the total cases should now be equal to the number of processed cases. For example, if I have 3500 Total Cases, and if I run the breakout process, the Processed Cases should now be 3500.

Am I missing something here?

See this case or this case on the forums. It may be that the dictionary has changed since data collection. If so, you should allow default value of any added field to be null or an empty string, so that all cases before and after the changes are compatible.
